Note: O Levels students will appear for Sports and co-curricular trials with Matric students.
Follow the instructions below for an easy form submission:
- Use capital letters to fill the form
- Note your Tracking code when on the 2nd page of the form
- Once finished, print the form and click Submit
- Submit the printout in Ahmad Saeed Administration Building room # 217
- Attach all the following documents with the printed form:
- 4 unattested passport size photographs
- 2 attested photocopies of the provisional result intimation (result card)
- 4 attested photocopies of good conduct certificate
- Computerized bank receipt of Rs 500Processing fee of Rs 500 must be deposited in any Bank Al-Falah branch using a cash deposit slip:
